Instructions to Find Primary School Location
Click on the school calendar link below to make an appointment for registration between January 20-30, 2026
After you have made your appointment, please review the required forms BELOW to ensure that you have all of the completed forms for the date of your appointment.
Required Forms for Incoming Kindergarten Students
The Packet Consists of the Components Below and They Are All Required:
- Home Language Questionnaire - To be completed by parent
- Required NYS School Health Examination Form- To be completed by a physician
- Student Health History- To be completed by parent
- Authorization to Release Confidential Information- To be completed by parent
Make sure all forms are completed, incomplete forms will not be accepted
New York State Immunization Requirements for Kindergarten Entry Proof of Immunization Must Be Provided at Time of Registration
- Diptheria, Pertussis, and Tetanus Vaccine (DTaP, DPT) 4-5 doses. One dose must be given after the 4th birthday
- Polio Vaccine (IPV/OPV) 3-5 doses. One dose must be given after the 4th birthday
- Hepatitis B Vaccine 3 doses
- Measles, Mumps, Rubella (MMR) 2 doses. First Dose on or after first birthday
- Varicella Vaccine (or MD proof of disease) 2 doses. First dose on or after first birthday
NOTE: Proof of immunization must be submitted upon registration. Your child may not begin school without complete immunization records.
